I’ve seen this reported a number of times with different characters.
I’ve had it happen once with my Zane, so it’s not a Fade Awayissue, it’s a bug in general. Guessing it’s tied to the charged overlay, but could be wrong, if it were my code that’s where I’d look first.

I have. Just finished Story mode again and during the travels to the Heart I lost buff and had to re-stack. Didn’t count how many kills since Clone and Drone were always active so I don’t know who killed what.
