I got an exclamation mark on “Play”, then “Mode: TVHM” eveytime I start the game…
What does that means?! 
If you beat it once it is giving you the option for a second playthrough in tvhm.
well… I beat it twice… the exclamation mark after I beat it twice and doesn’t go away! even after restarting the game! :o
It’s about the new option of resetting all of the quests after finishing your TVHM main story playthrough.
You can safely ignore it.
