Few corrections: Those “Like, Follow, Obey” head/skins are literally as it’s been said: early access for streamers. They will be released later to rest of the players.
Tuned and Doomed and Skull and Domes heads are from pre-order packs, not from Bloody Harvest event. Those (too! 
) were bugged and were silently fixed on patch that just happened to came out around Halloween event. Bloody Harvest heads are Spooky Surprise ones, as listed.

Caustic Bloom skins are mission “The First Vault Hunter” rewards.
And finally, multiple skins seem to be plain old world drops:

Are you sure? I know only about one head (each VH), and it was added to the game on Oct 24th.
Yeah, JoltzDude confirmed this. He further revealed that they will be free addition later date.
Edit: indeed, they were added to game in October but without legit way to obtain them until later. Well, some players seem to have had “positive bugs” and received them already? Still, any of them have not told where they obtained these customizations. Suspicious? 
Well so annoying, cause it’s over two months now. It would look better if total number of customizations wasn’t increased for all the players.
I agree. It was similar issue in BL2, total numbers also included skins/heads that could not be obtained (especially PS4/XB1 with Handsome Collection): for example Poker Night/Diamond Loot Box/Special Edition (BL1 save reward) heads and Gearbox employee skins. Plus Vermi heads/skins for singleplayer 
Note that there’s also couple BL3 vehicle skins that are no longer available (gift from E3 2019) and probably one named Gearbox.

P.S.: I’m a fan of the series since BL1. I’m patient(ish still?) about BL3, about bugs and all the stuff going on, but for the first time, I think I have to say, it’s not fair giving out skins like the ones on E3. For BL2 I said, okay, I bought the game too late, I did not have the Poker Night game anyway… Fine, I get with it. But how in the h*ll I’m supposed to get to E3 from like cca. 6200 miles from L.A. to a venue which can take in a limited of guests anyway? About the Diamond/Special loot chests… The only thing I have to say, that in Hungary, where I live, it isn’t available at all. I know it’s super limited and stuff, but I’m sure they could made some solution for it. Like taking 50 orders, 100, and produce it after that. Take another 100 and produce it. The closest country for me to buy it, would have been France with 770 miles. But you can always buy from Ebay for double the price right?

New head and new skin in today’s update. Skin is from event, don’t know about head.
Thanks to the feb 13th patch they made the NOG head and a few skins obtainable
- “NOG Mask” Head- Rewarded For Completing “Techincal NOGout” in Meridian Outskirts
- “Atlas Classic” Character Skin - Rewarded For Completing “Atlas, At Last”
- “Gilded Rage” Character Skin -Rewarded For Completing “Cold as the Grave”
- “Signature Style” Character Skin - Rewarded For Completing “The Great Vault”

Spreadsheet’s been updated with the new cosmetics, btw. In addition to the new Heartbreaker skins, there’s also a new set of heads hiding in the data, which are presumably going to be used as Twitch Prime rewards in a bit (given that “Twitch” is in the object names). Named “Roll Player” for all characters, basically a d6 with the “5” as the face, with some bunny-ear type things on the back. Spreadsheet has the balances, so if you’re doing any CE-based item-drop shenanigans, that’ll get 'em for you.

Unlocking the Heartbreaker skin with one character will unlock it for every VH.

Oh, interesting! Good to know – the Bloody Harvest rewards required you go to through once with each character.

Wait, do you get the roll player and the streamers heads? How? I want them so bad 
The streamer heads are only for those who were physically at PAX East show. They get a code. The Roll Player probably will be a Twitch Prime loot.
I feel a little robbed. Its a little unfair for all of us we can’t go to things like the PAX :c . Hope they put It on sale at least.
I’m not sure it will be for sale. There doesn’t seem that there will be any micro-transactions.
