I’ve only had it happen once so far. Playing on the Xbox One, what I believe happened was, while I was trying to revive a team mate, I died. I believe this caused the (not sure what to call it) white swirl stuff that soars around the character to remain on the map after my team mate had respawn. I’ve yet to try and replicate this glitch.
