I’m currently 42 lvl. I’ve finished the story missions (not including dlcs). And I skipped most of side quests. Should I buy Mayhem lvl 10 for finishing all the side quests? Or should I first get lvl 1 Mayhem and increase it slowly? Thanks in advance.

At level 42 i would suggest proably M4-6, if you get something like a Hellwalker you can jump to M11.

The advice above is reasonable. I see very few people going level-by-level: it seems like most people play either at the bare minimum to get level scaling in NVHM (1/2), 4-6 (I think 6 unlocks some unique drops?), or 10/11. The issue being that each level has a set number of hard/medium/easy modifier categories, so the scaling isn’t just a simple difficulty hike.

You can also samplethe Mayhem levels/rolls as you like; they can be changed mid-game from the menu (the icon with the horned skull).
Jump straight to MM10, travel somewhere fun, and see what happens. If that’s too nutty, you can change the mode right in the middle of the fight (which will reload the map).

go to sanctuary set mayhem to 10
use golden keys then go kill stuff at mayhem 10 or 11
lower mayhem levels get you stuck at that level because once you find a a great m5 gun it will be much less great at m6

I am going to contradict everyone here:
At level 42 I wouldn’t worry about Mayhem on NVHM at all.
Go to TVHM and play through the main story again and level up your character as far as you can, doing everything but the Crew Challenges so you can get those at max level later. Do all the mission content until you hit max level.
Then, and only then, do you go back to Normal Vault Hunter Mode and experiment with Mayhem.
The most impactful thing about Mayhem is what Modifiers you play with. The odds for good ones vs. bad ones varies with each mayhem level and they have a good illustration of which level has which type at www.vaulthunterhub.com
The levels themselves impact HP, XP, and Loot, but the overall difficulty of the game is going to vary, and Modifiers impact that so heavily that not optimizing which ones you play with can make the game unenjoyable if not downright unplayable.
There is exclusive gear that unlocks at Mayhem 4 and more that unlocks at Mayhem 6 if that’s what you’re interested in. I usually play at MH 4 until about level 57, then jump into 11 as I have saved level 57 MH 10 gear such as the OPQ/backburner/sand hawk.

This was actually my original opinion. Reach max level then worry about Mayhem levels. The only thing OP will miss out on is a substantial EXP boost.

First time into Mayhem I went with gradual approach: M2, M4, M6, M8 - mostly trying to minimize number of modifiers to deal with.
On every subsequent character I went straight to M11 and simply considered early struggles part of the challenge (to be fair, having a bunch of hand me downs in the bank helped a lot, and so did decent Guardian Rank I’ve had by then).

Set Mayhem lvl11, jump around on stations, buy legendary weapons and gear from vendor machines, run to the weekly maurice vendor machine and buy the recommended legendaries… enjoy game on hardest level until you get bored.
I just collect the best weapons now, but not much to do other than that to be honest.
