That unfortunately happens in a LOT of places: the bank vault room and the fast travel station have both eaten gear on me, as have random rocks and cliff faces.
@VaultHunter101: It’s one of the main reasons I gave up on TPS. Losing it through the wall was bad enough – it dropping through the floor [=flat ground!] was WTF. Granted that this problem existed in B2, but it thankfully didn’t happen nearly as commonly.
I’ve had that happen a few times as well. I guess he had an important appointment somewhere.

I’ve seen Goliath do that before they just kinda fly up to beyond the stars.
I’ve seen a video of Bunker doing the exact same thing. That, and him doing his lootsplosion way out in the far distance…
I looked on it as a form of justice for the player recording the video simply killing Bunker too quickly - if the fight was really that easy, you shouldn’t get such a large reward 
Now imagine having that happen with OP8 Vorac at the end of a grueling fight. Two times straight.
Melee characters can still get him if they’re quick about it, but Maya isn’t a melee character.
Dammit.
The physics in this game can be Unreal at times!

Was farming hyperius yesterday and during his across the map jumps at me I looked back and he was gone? All the bots were still present and you could see them all feeding hyperius his shield but he somehow glitched under ground in the far left corner as you enter. I could still kill him but no loot popped up. 
Not a bad glitch (didn’t see a good one) but once i fast traveled with a buddy while ending or starting Krieg’s badass transfomation, and on the other side i was still a badass. I could use all my weapons, My midget hand couldn’t reach any of my guns (assult rifle, smg, etc). It looked pretty hilarious when my buddy sent me pictures. we couldn’t repeat it sadly.

I don’t think that’s a glitch, just a really unlucky (for you) spawn. RNG is fickle and cruel.

Just had one happen to me today. I’d say it’s the worst glitch ever for me, because it’s the only one I’ve ever had that I couldn’t find a way around and it happened at the worst possible moment. So, I was finishing TTAODK with my Zer0, going through the Lair of Infinite Agony doing A Game of Games, The Amulet, My Dead Brother and Post-Crumpocalyptic simultaneously just so I wouldn’t have to run the entire map again. That place is a pain in the ass to go through and I even exited and re-entered a couple of times to make sure I had the exclamation point for The Amulet active before starting. All was going well and I had reached the second area where you’re supposed to resurrect and kill skeletons for the My Dead Brother quest. I cleared the entire area of enemies, even all those annoying Skeleton Mages. That was a lot of work and took quite a while. Then I go hit the sword to start the mission objective. The sword goes into the ground and… Nothing happens. No skeletons come out of the ground, the mission objective stays uncompleted and the waypoint for it stays fixed on the ground. I open another area, kill some enemies, go back and nothing happens. I switch to another mission and then back again and nothing happens. I shoot and melee at the sword and nothing happens. I’m literally midway through the map with a lot of other missions half completed, so I can’t save and quit or return to the entrance to exit and re-enter the place. It would be too much work to run that portion of the map again, I was afraid of losing progress on the other missions and I could lose the Amulet mission if I did that. So I just decided to keep going anyway and ignored My Dead Brother altogether. Now I’m finished with the DLC and that’s the only mission I have left. I’m gonna try it again tomorrow and I really hope the save and quit has fixed the glitch. Otherwise, that mission is going to stay open forever unless I reset the entire playthrough, and I really don’t wanna do that at the moment. Please, tell me I won’t have to do that.

You may be able to complete it in someone else’s game. Might even be easier to do it that way.
Kind of just had that happen to me on the Get Shorty quest for Scooter. I killed Shorty and the stalker that had swallowed him from Deception with a melee attack rather than using a shock weapon. The mission remained active until I quit, saved and restarted the mission so I’m guessing you should be fine. Almost had something like that happen again while doing the 3:10 to Kaboom mission in Lynchwood- for some reason I got a ‘full’ message when trying to place the bomb in the mine casrt. I was running short on time so I decided to pop Deception- it seemed to fix the problem as I was able to put the bomb in the cart after Deception wore off…
Thanks for the replies, @VaultHunter101 and @Carlton_Slayer. I tried it again today and the save and quit reseted the sword, so I was able to complete the mission without any further hiccups.
